Most of my patients who are dealing with low back pain will have a pattern of pain that is episodic in nature. This means that the pain will come and go, sometimes with no obvious reason. Many times my patients will report that their pain has completely resolved during a visit, then by the next it has returned with a vengeance. We see many times that the back will feel better which gives them the confidence to do yard work. They end up going too hard on hard work which causes them to hurt their back again. It is helpful when patients come back so that we can address the new pain and dispel any concerns about the recent exacerbations. With home therapy that emphasizes prolonged care with maintenance programs built into them, we can see those patients quickly when pain arises and reduce the amount of time it takes to return to a pain free back.
For example, I had a patient who was dealing with chronic low back pain for a number of years. He would get pain and numbness when he would walk for about 5 minutes. Using interventions to address lumbar stenosis, we were able to increase the time it took for his symptoms to come on. We also taught him techniques that allowed him to walk all day while at an amusement park. This was an interesting case because we weren’t able to eliminate his low back pain or even the symptoms that went down the leg. What we were able to do was give him the opportunity to be able to walk for longer before the pain came on. He started at around 5 minutes and was able to build up to over 30 minutes. When the pain did come one, he would use a simple stretch of bending over several times to reduce the symptoms, and allow him to walk again. This is a great example of maximizing a patient with low back pain’s potential.
